The Case Re-review Request Details page allows you to enter supporting notes and/or upload documents in support of your re-review request.
This page displays:
Case Number: The Case Number for re-review.
Reason for Re-Review Request: The reason for re-review selected (Option 1 or Option 2 from the Case Re-Review Request page).
To continue, you must explain the reason for your re-review request. You can use the text box provided or upload documentation to support your request. If you submitted a re-review request because you have additional evidence not previously considered by CMS, or you believe a submission error exists in the documentation previously submitted that leads to a change in pricing of no less than $2,500 (Option 2 on the previous page), then you are required to upload documentation to support your request.
Notes: Free-form text
This section allows you to add free-form text to explain the reason for your re-review request. Notes have a 500-character maximum.
Documentation Upload
This section allows you to upload supporting documents, and also displays a list of documents that have already been uploaded.
Notes: If you selected Option 2 on the Case Re-Review Request page, you are required to upload supporting documents.
If any file cannot be uploaded, you will receive an error message. If you receive an error message, none of the files will be uploaded. You must correct the problem(s) and choose new, error-free files to upload. The error "Invalid File Size" may be related to the page size.
How to Add Documents
- From this page, click Upload Documentation. This opens the Attach Documentation page.
- From the Attach Documentation page, browse your system and select documents to upload to the case.
Attached documents then display on this page under the Supporting documentation uploaded heading. Selected documents must be in .PDF format, cannot exceed 40MB, and the file name cannot exceed 80 characters. Page size must be no larger than 8.5 x 11 inches. - To delete a document, click Delete to the right of the file name. This permanently removes the document from the WCMSAP.
Note: Once documents are added to a case, the contents of that document CANNOT be viewed. Review the document on your system prior to uploading to ensure its accuracy.
Click Previous to return to the Case Re-Review Request page.
Click Continue to continue with the request and display the Case Re-Review Request Verification page.
Click Cancel to cancel the re-review request and display the Cancel Re-Review Request Confirmation page.
