Medical Care

The Medical Care page allows you to look up major medical centers by ZIP code.

When creating a case, you can use this tool to identify the major medical center for a beneficiary as part of a Workers' Compensation Medicare Set-Aside (WCMSA) proposal, should the expected future treatment for the work injury, illness, or condition require the use of one.

To begin, select Yes or No to the question, "Does the proposed WCMSA for this settlement include any costs associated with a major medical center?"

ElementDescription
Zip CodeEnter a five digit numeric ZIP code for the major medical center used in the calculation of the proposed WCMSA.
Required when Yes is selected confirming associated medical costs.

Click this button to search for the major medical center that Medicare is associated with the ZIP code.
If no match is found, a message is shown. If you entered a valid ZIP code and didn't get any results, please contact an Electronic Data Interchange (EDI) Representative by phone at (646) 458-6740 or email at COBVA@bcrcgdit.com. for assistance.

Major Medical Center ResultsDisplays the information about the major medical center based on the ZIP search entered. The information is read-only.
Zip CodeDisplays the ZIP code provided for the search.
NPIDisplays the National Provider Identifier number of the major medical center.
FacilityDisplays the name of the major medical center.

Click Previous to return to the previous page.
Click Next to save all information and proceed to the next page.
Click Case Summary to view a synopsis of the case information entered.
For new cases and cases in Work-In-Progress (WIP status):

  • Click Save Work-In-Progress to save all information entered up to this point. You can return to the WCMSAP later to complete the case creation process.
  • Click Cancel Case Creation to delete all information entered and cancel the case creation process.
