Use this page to search for a diagnosis code to add to a case. Search for codes by entering either a partial or full diagnosis code, or by entering descriptive keywords. Click Clear next to either field to clear any value you have entered. Click Search next to the field you use to find all diagnoses that include the text or numbers entered. Enter at least three characters in a field.
You CANNOT search by Diagnosis Code and Diagnosis Keyword at the same time. Each type search must be conducted separately.
Diagnosis Code: Enter a partial diagnosis code (at least three characters). Click Search to display code matches,

Diagnosis Keywords: Enter a descriptive keyword(s). Click Search to display code matches.
A list of diagnosis codes displays near the bottom of the page. Select the desired diagnosis code by clicking the radio button next to the code. Click Select Diagnosis Code to transfer the code to the Diagnosis Codes page and add it to the case. Return to the Search Diagnosis Codes page if you need to look for additional codes.
Note: You may not mix ICD-9 and ICD-10 codes on one case, so if there are ICD-10 codes on a case you may not add ICD-9 codes to it, and vice versa. Cases with an initial Date of Incident (DOI) of 10/1/2015 or later must exclusively use ICD-10 codes.
Click Cancel to return to the Diagnosis Codes page. No codes will be added to the case if you select Cancel.
