This page lets you specify which cases associated with your Login ID to display. Use the fields on the page to narrow search results.
Select the All Cases radio button to view all submitted and Work-In-Progress cases associated with your Login ID.
Select the Submitted Cases Only radio button to view all submitted cases associated with your Login ID.
Select the WIP Cases Only radio button to view all Work-In-Progress cases associated with your Login ID.
The following fields are optional.
- Case Control Number - Enter a Case Control Number to narrow results.
- Medicare ID - Enter a Medicare ID (Health Insurance Claim Number [HICN] or Medicare Beneficiary Identifier [MBI]). (The Medicare ID is also known as the Medicare Number to CMS' Medicare beneficiaries),
OR
- SSN - Enter a Social Security Number.
- Case Creation From Date - To enter a case creation date range, enter a beginning case creation date.
- Case Creation To Date - Enter an ending case creation date.
- Case Submission From Date - To enter a case submission date range, enter a beginning case submission date.
- Case Submission To Date - Enter an ending case submission date.
Click the Search button to display cases using your search criteria. Click the Clear button to empty all fields.
Click the Cancel button to return to the Home page.
