The Beneficiary/Claimant Attorney page allows you to add the beneficiary or claimant’s attorney information to a case. This page is optional. Do NOT enter any information on this page if the beneficiary or claimant is not represented by an attorney.
If information is entered in any field then the following fields are required: Last Name, First Name, Address Line 1, City, State, Zip Code and Phone.
Beneficiary/Claimant Attorney Data Elements

| Last Name | Enter the attorney's last name. |
| First Name | Enter the attorney's first name. |
| MI | Enter the attorney's middle initial. |
| Address Line 1 | Enter the first line of the attorney's mailing address. |
| Address Line 2 | Enter the second line of the attorney's mailing address. |
| City | Enter the city where the attorney is located. |
| State | From the drop-down list, select the state in which the employer is located. |
| Zip Code | Enter the attorney's ZIP code. |
| Phone | Enter the attorney’s phone number. |
| Fax | Enter the attorney's fax number. |
| Attorney Email Address | Enter the attorney's email address. |
| Re-enter Attorney Email Address | Re-enter the attorney's email address. The address must be manually entered. The address cannot be copied and pasted. |
Click Previous to return to the previous page.
Click Next to save all information entered and proceed to the next page.
Click Case Summary to view a synopsis of the case information entered.
For new cases and cases in Work-In-Progress (WIP) status:
- Click Save Work-In-Progress to save all information entered up to this point. You can return to the WCMSAP at a later time to complete the case creation process.
- Click Cancel Case Creation to delete all information entered and cancel the case creation process.
