This warning page appears for Corporate or Representative account types when the information entered on the New Case Creation page cannot be matched to an existing Medicare beneficiary on record after clicking Continue.

Click Previous to return to the New Case Creation page. The information you entered previously redisplays.

Click Continue to proceed to the Beneficiary/Claimant Information page. On this page, you can enter information about the claimant that will be used in the new case.

If the claimant has a reasonable expectation of becoming a Medicare beneficiary within the next 30 months (2 years, 6 months), you may proceed with creating a new case.

Click Cancel to return to the Home page. All information you entered will be lost.

Note: You may cancel the case creation process at any time before you submit the case. If you save the entered information as a Work-In-Progress, you may return to the case in the future and continue entering information. If you do not save the case as a Work-In-Progress, cancelling the case creation process will cause all information entered to be lost.
