Note: If available, the beneficiary's recorded First name, Middle initial, Last name, Medicare ID (Health Insurance Claim Number [HICN] or Medicare Beneficiary Identifier [MBI]) or Social Security Number (SSN), Date of birth, and Gender will be displayed as read-only on the Beneficiary/Claimant Information page. The first five digits of the HICN or SSN will be masked with asterisks.
Click Cancel to return to the Home page. All information you entered will be lost.
Note: You may cancel the case creation process at any time before you submit the case. If you save the entered information as a Work-In-Progress, you may return to the case in the future and continue entering information. If you do not save the case as a Work-In-Progress, cancelling the case creation process will cause all information entered to be lost.
