The Alert Lookup page lists all alerts associated with the Account ID(s) you are registered under. The default view is alerts within the previous 60 days that are not archived. Use the fields on the page to filter the displayed alerts.
| Case Control Number: | Enter a Case Control Number to narrow results. |
| Alert Status: | Select Read, Unread, Archived, Not Archived or All. |
| Medicare ID: | Enter a Medicare ID (Health Insurance Claim Number [HICN] or Medicare Beneficiary Identifier [MBI]); (the Medicare ID is also known as the Medicare Number to CMS' Medicare beneficiaries), OR |
| SSN: | Enter a Social Security Number. |
| Alert Creation From Date: | To enter an alert creation date range, enter a beginning alert creation date. |
| Alert Creation To Date: | Enter an ending alert creation date. |
Click the Search button to narrow the displayed alerts using your search criteria. Click the Clear button to empty all fields and show all alerts. If a date range is not provided, the application will display alerts for the current 60 days.
Each alert lists the following information:
Alert ID – The sequential number of the displayed alerts. Click the Alert ID link to display the Alert Detail page and view more information about the alert. (The ID is not linked for alerts for deceased beneficiaries.)
On the Alert Detail page, you can view the alert itself (usually a letter), or click the Archive this Alert button to archive the alert, or click the Close this Window button to close the alert and return to the previous page (Alert Lookup or Alert Listing).
Alert Creation Date – Date the alert was created.
Alert Type – Type of alert related to the case. Most alerts are accompanied by a letter, and contain the associated letter's contents. Options are:
- Under Threshold
- Development
- Deny
- Zero Set Aside
- Approval (includes Recommendation attachments)
- Closeout
- Deceased Beneficiary (does not produce a letter)
- Commingled Document (does not produce a letter)
- Commingled Follow-up (does not produce a letter)
- Updated Identifier
- Re-Review Request Decision - Approval
- Re-Review Request Decision - Denial
- Amended Review Decision - Approval
- Amended Review Decision - Denial
Case Number – Case Control Number associated with the alert. Click the Case Number link to display all alerts for that case.
Creator – Entity that entered the alert. Options are Submitter, WCRC, and RO.
Status – Status of the alert.
Medicare ID/SSN – The Medicare ID (HICN or MBI) or SSN of the beneficiary associated with the case. The first part of a HICN and SSN is masked with asterisks.
Bene Name – Name of the beneficiary associated with the case.
Click the Cancel button to return to the Home page.
