Warning - Confirm Request to Submit a Waiver

Submitting a request for a waiver means you are requesting to have Medicare waive, or dismiss, the recovery of the demand amount owed, in full or in part. The waiver submission process requires you to submit documentation to back up your request. It is recommended that you complete and submit the SSA 632 Request for Waiver form (link provided on the next page), which requests specific information about your income, assets, expenses, and the reasons why a waiver of recovery should be granted.

If you are an authorized user and select Submit Waiver Request, you will see this warning page. It is important to understand the differences between a redetermination, waiver, and compromise so that you submit the proper request. Read this page entirely in order to determine if you meet the qualifications or conditions described.

If you decide to submit a waiver request, click Continue to proceed to the Waiver Submission page; otherwise, click Cancel to return to the Case Information page.
