Redetermination (First Level Appeal) Documentation Upload

The Redetermination (First Level Appeal) Documentation Upload page provides you with the ability to upload documentation to support a redetermination request.

Requirements for Uploading Documents

The MSPRP requires each uploaded file to follow the requirements listed on the upload page itself.

To Upload a File

To upload a file from your computer to the MSPRP.

  1. Click Choose File to search your computer for the desired file.

    A pop-up box appears. Locate the file that you want to upload.

     

  2. Click the file name, and then click Open.

    When the file has been selected, the file name and location will appear on the Documentation Upload page.

    If you have more than one file to upload, repeat the previous step. Each new file must be entered in the next available text box. You are limited to uploading five files at a time.
    Note: If you have more than five files to upload, you must return to this page from the Redetermination (First Level Appeal) Verification page and repeat the upload process.

     

  3. Once all files have been identified, click Continue to upload the documents.

    If you receive an error message, none of the files will be uploaded. You must correct the problem(s) and choose new, error-free files to upload. Note: The error "Invalid File Size" may be related to the page size, which must not be larger than 8.5 by 11 inches.

    If the upload is successful, the Redetermination (First Level Appeal) Verification page will appear. The name of each uploaded file will appear on the bottom of this page.

Click Cancel to return to the Redetermination (First Level Appeal) Verification page without uploading any documents.

You may click the Print this Page link to print a copy of this page for your records.
