Case Documentation Upload

The Case Documentation Upload page allows you to upload documentation associated with your case.

Requirements for Uploading Documents

The MSPRP requires each uploaded file to follow the requirements listed on the upload page itself.

To Upload a File:

To upload a file from your computer to the MSPRP:

  1. Click Choose File to search your computer for the desired.

    A pop-up box appears. Locate the file that you want to upload.
     
  2. Click the file name and then click Open.

    When the file has been selected, the file name and location will appear on the Documentation Upload page.

    If you have more than one file to upload, repeat the previous steps. Each new file must be entered in the next available text box. You are limited to uploading five files at a time.

    Notes: If you have more than five files to upload, return to this page from the Submit Case Documentation page and repeat the upload process.

    Only documentation for the selected document type should be uploaded. To ensure proper handling, other requests must be submitted by returning to the Submit Case Documentation page, selecting the applicable document type, and uploading the related documentation (CRC cases only).
  3. Once all files have been identified, click Continue to upload the documents.

    If you receive an error message, none of the files will be uploaded. You must correct the problem(s) and choose new, error-free files to upload. Note: The error "Invalid File Size" may be related to the page size, which must not be larger than 8.5 by 11 inches.

    If the upload is successful, the Submit Case Documentation page will appear. The name of each uploaded file will appear on the bottom of the page.
     

    Click Cancel to return to the Submit Case Documentation page without uploading any documents.

    You may click the Print this Page link to print a copy of this page for your records.
