This page is used to capture information for a beneficiary associated with a case(s) that will be accessed using this Medicare Secondary Payer Recovery Portal (MSPRP) account. All fields are required.

Note: The Social Security Number (SSN) is only required if the Medicare ID (Health Insurance Claim Number [HICN] or Medicare Beneficiary Identifier [MBI]) is not provided. If you enter both the SSN and the Medicare ID, you will receive an error message.

Information entered in this section is for a beneficiary associated with a case(s) that will be accessed using this MSPRP account.

Beneficiary Information

Field	Description
Beneficiary Last Name	The beneficiary's surname as it appears any case specific correspondence received from CMS. At a minimum, you must enter at least 5 characters.
First Initial	First letter of beneficiary's first name.
Medicare ID	The beneficiary's HICN or MBI. The Medicare ID is CMS's Medicare identifier for Medicare beneficiaries and is the preferred data element for matching purposes.
Beneficiary Social Security Number (SSN)	The beneficiary's Social Security Number. This number must be unique in the MSPRP. It cannot be the same as the Social Security Number entered on the Representative Information page. The SSN is required if the Medicare ID is not entered.
Beneficiary Date of Birth	The beneficiary's date of birth.
Beneficiary Gender	The beneficiary's sex. This is selected from a drop-down list.

Click [Continue]to proceed to the next page in the New Registration process which is the Registration Summary page.

Click [Cancel] to cancel the New Registration process and return to the Welcome to the MSPRP (Login) page.

Next Steps

The MSPRP will attempt to determine if the entered information can be matched to a Medicare beneficiary. The system must first find an exact match on the Medicare ID or SSN. Then at least three out of four of the following fields must be matched exactly: First 6 characters of the Last Name, First Initial, Date of Birth, and Gender. If a match is not found, you will receive an error message that states, "Data does not match what is on our records for the Beneficiary". You will not be able to proceed with registration.
