PIN Request Completed Successfully Thank You

The PIN Request Completed Successfully Thank You page provides final confirmation that the Personal Identification Number (PIN) Request process for the CRCP has been completed successfully. Your account ID is included on this page; therefore, it should be printed and kept with your records. An email containing the account ID will also be sent to the account representative (AR). Please print this email for your records; this information is needed for account setup.

Next Steps

Once the CRC has vetted your information, an email will be sent to the named Authorized Representative (AR) at the email address listed on the PIN Request. This email includes the PIN and is titled "CRCP PIN # Notification."

Account Setup

After receiving the account ID and the email containing your PIN, the AR must give this information, and company contact information, to the designated Account Manager (AM) for your company. The AM is the one who must complete the Account Setup process.

To begin the Account Setup process, the AM must go to the Welcome to the CRCP page at https://www.cob.cms.hhs.gov/CRCP/ and click the Step 2 - Account Setup button.
