The Account Designee Personal Information page allows an invited Account Designee (AD) to enter personal contact information as part of the AD registration process. This information serves to identify you as a user for the Commercial Repayment Center (CRC).
To Complete AD Personal Information
Complete the following fields. All fields are required except where noted.
- First Name
- Middle Initial (MI) (optional)
- Last Name
E-Mail Address (read only)
If you need to modify your e-mail address, you can do so only after completing the registration process. Once you are registered, click the Update Personal Information link under the User Options menu.
- Phone (Area Code, Exchange, Suffix)
- Extension (Ext) (optional)
- Address Line 1
- Address Line 2 (optional)
- City
- State
- Zip Code
Click Continue to continue to the Account Designee Login Information page, or click Cancel to cancel the registration process and return to the CRCP Login Warning page. No data is saved.
