The Update Authorized Representative (AR) Information page allows the Account Manager (AM) to update the AR’s contact information, or to enter new information to replace the AR, if necessary.
The fields are pre-populated with the information currently stored on the CRCP. You may make edits to any field.
To Update AR Contact Information
All fields are required unless otherwise noted.

- MI (Middle Initial) (optional)
- Last Name
- Title
- E-Mail Address
- Re-enter E-Mail Address
- Phone (Area Code, Exchange, Suffix)
- Extension (Ext) (optional)
- Fax (optional)
Click Continue to save the changes and continue to the Update Authorized Representative (AR) Information Confirmation page, or click Cancel to cancel the updates and return to the Account Detail page.
